ASSUMPTION OF RISK AND WAIVER OF LIABILITY RELATING TO COVID-19
The novel coronavirus (“COVID-19”) has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. Attending an in-person program could increase your child(ren)’s or your risk of contracting COVID-19. Mega Academy Inc. cannot guarantee that you or your child(ren) will not become infected with COVID-19.
I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the in person program and that such exposure or infection may result in personal injury, illness, permanent disability, and death.
I understand the risk of becoming exposed to or infected by COVID-19 at an in-person program held by Mega Academy Inc. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience.
NOTICE TO PARENTS
Dear Parents,
In order for students to have a good learning environment, we advise parents to cooperate with the following regulations.
-
Each registered student will be enrolled in a class on Google Classroom. The purpose of the Google Classroom is to provide materials for the student to use during class sessions and for homework. All grades will be recorded on Google Classroom and will serve as a way for parents to know how the student is doing in class. In order to monitor the student’s achievement, parents are responsible for checking the Google Classroom regularly.
-
Homework is assigned during each class session and due the following class session. Late homework will not be accepted without an explanatory note from the parents.
-
Please notify the office of any special conditions (including medical conditions) that the student might have. Parents should leave emergency contact phone numbers (ex. Doctor’s name and phone number) with the office if such conditions do exist.
-
